Montana Unemployment Application?

I'm applying for Montana Unemployment. Can I be disqualified for gross misconduct?
First it would be nice to know what is gross misconduct.
My employer stated I was fired for not being in touch with them within 48 hours after letting them know I was sick. They also said I was fired for insufficient job activity. I don't understand. Do either of these reasons jeporadize my right to collect unemployment?
Where can I find the unemployment application?

Misconduct is when you ignore your boss, do as you please, you know.
This kind of action is negative for you but I consider your employer's decision pretty extreme.
